HEALTH INFORMATION
The hospital collects personal health information about patients
from the patient or the person acting on a patients
behalf. The information may include a patients name,
date of birth, address, health history, records of the hospital
visits and care received during those visits. On occasion,
personal health information is collected from other sources
should the patient give consent or if the law permits.
Uses
and Disclosures of Personal Health Information
The hospital may use and disclose patient personal health
information to staff, students and physicians for:
treating and caring for a patient.
payment services provided (from OHIP, WSIB, your private
insurer or others).
planning and administering internal operations.
conducting quality improvement activities (such as
patient satisfaction surveys).
compiling statistics.
fundraising to improve our healthcare services and
programs.
complying with legal and regulatory requirements.
fulfilling other purposes permitted or required by
law.
Patient
Choice
A patient may access and correct their personal health records
or withdraw consent for some of the above uses and disclosures
by contacting the hospital.
Important
Information
The hospital takes steps to:
protect personal health information from theft, loss
and unauthorized access, copying, modification, use, disclosure
and disposal.
conduct audits and complete investigations to monitor
compliance with privacy requirements.
ensure everyone performing services protects the privacy
and only uses personal health information for the purposes
to which consent has been given.
